NAD+ IV therapy can cause temporary infusion-related side effects, particularly when the infusion is administered faster than an individual comfortably tolerates. Reported experiences include nausea, abdominal cramping, diarrhea, vomiting, flushing, headache, throat discomfort, chest pressure, increased heart rate, fatigue, and lightheadedness. Human evidence on long-term NAD+ IV safety and effectiveness remains limited.
A 2026 real-world pilot study comparing intravenous NAD+ with intravenous nicotinamide riboside found noticeably poorer tolerability in the NAD+ group. Participants receiving NAD+ reported gastrointestinal symptoms, increased heart rate, throat discomfort, and chest pressure during infusion. The study was small, so it should not be used to predict what every patient will experience, but it reinforces the importance of careful clinical supervision and infusion tolerance.

The 2026 retrospective pilot study was very small—only six participants received NAD+—but all six reported moderate-to-severe abdominal cramping, diarrhea, nausea, vomiting, increased heart rate, throat discomfort, congestion, and chest pressure during their infusions. Symptoms resolved after the infusion ended, and discomfort led participants to slow the infusion.
That does not mean every NAD+ IV patient will experience those symptoms. It does mean that NAD+ IV should not be marketed as though it is always a quick, symptom-free hydration drip.
Important: Chest pain, breathing difficulty, fainting, facial or throat swelling, severe vomiting, severe abdominal pain, or rapidly worsening symptoms should not be dismissed as a “normal NAD reaction.” Seek appropriate medical evaluation.
What Is NAD+ IV Therapy?
Nicotinamide adenine dinucleotide, or NAD+, is a naturally occurring coenzyme found in human cells. It participates in important biological processes involving:
- cellular energy metabolism;
- redox reactions;
- mitochondrial function;
- DNA-related cellular processes;
- and cell signaling.
Interest in NAD+ has increased because NAD biology changes with age and is involved in many metabolic pathways.
But there is an important distinction between NAD+ being essential to normal cellular biology and intravenous NAD+ having proven clinical benefits for a particular health condition. Those statements are not equivalent.

Why Can NAD+ IV Cause Side Effects?
There is no single explanation for every symptom experienced during an NAD+ infusion. However, one of the most important practical factors appears to be infusion tolerance and pace.
Infusion Pace Matters
NAD+ IV is not necessarily experienced like a standard saline hydration infusion. Some people may tolerate the infusion comfortably, while others report discomfort that limits how quickly the infusion can be delivered.
In the 2026 pilot study, NAD+ participants required an average infusion time of approximately 97 minutes, compared with 37 minutes for the nicotinamide-riboside IV group. The longer duration was related to infusion-related symptoms and participant-directed slowing of the NAD+ infusion.
The study does not establish an ideal infusion duration for every patient. It does, however, support an important principle: faster is not automatically better with NAD+ IV. The objective should be an appropriately supervised infusion that the individual can tolerate—not completing the bag as fast as possible.
More NAD+ Is Not Automatically Better
Higher amounts should not automatically be framed as more effective or more desirable. The appropriate discussion depends on:
- the reason someone is considering treatment;
- medical history;
- current medications;
- previous NAD+ exposure;
- symptoms;
- and clinician assessment.

Common NAD+ IV Therapy Side Effects
Nausea
Nausea is one of the more frequently described infusion-related symptoms in published NAD+ IV reports. In the 2026 pilot, nausea occurred alongside other gastrointestinal symptoms in the small NAD+ group.
If you become nauseated during an infusion, tell the clinician immediately rather than waiting for it to become severe. The treatment may need to be reassessed. Nausea should not be presented as evidence that NAD+ is “working.”
Abdominal Cramping and Stomach Discomfort
Abdominal cramping has also been described during NAD+ IV administration. Possible experiences may include:
- stomach tightening;
- abdominal discomfort;
- cramping;
- gastrointestinal urgency;
- diarrhea;
- or nausea.
Published evidence remains limited, but the 2026 observational study found substantial gastrointestinal discomfort among NAD+ recipients. Persistent or severe abdominal pain deserves clinical evaluation rather than being automatically attributed to the infusion.
Can NAD+ IV Cause Diarrhea or Vomiting?
Both diarrhea and vomiting were reported in the small 2026 NAD+ IV pilot. Again, the study population was too small to estimate how commonly these symptoms occur across the general population.
But these symptoms matter because repeated vomiting or significant diarrhea can contribute to:
- dehydration;
- weakness;
- dizziness;
- electrolyte disturbances;
- and worsening underlying health problems.
Tell your clinician promptly if these symptoms occur. Severe or repeated vomiting, severe abdominal pain, fainting, or inability to maintain fluids can require more urgent evaluation.
Flushing, Warmth, and Sweating
Some people describe warmth, flushing, sweating, or feeling temporarily overheated during IV therapy. These sensations can have multiple possible explanations and should not automatically be attributed to NAD+ itself.
If they occur during treatment, let the clinical team know. Any reaction that is rapidly worsening, accompanied by breathing problems, swelling, faintness, hives, or another concerning symptom requires more urgent assessment.
Headache and Lightheadedness
Headache and lightheadedness may occur during or following IV treatment. Potential contributors can include:
- hydration status;
- food intake;
- stress;
- blood-pressure changes;
- medications;
- an underlying medical condition;
- anxiety;
- or the infusion itself.
A 2024 systematic review of NAD-related interventions reported headaches and fatigue among adverse events, but this evidence should be interpreted carefully: the included randomized trials primarily investigated oral NADH or NAD precursors rather than NAD+ IV therapy. It therefore provides useful background but cannot establish the adverse-event rate for intravenous NAD+.
Persistent, severe, or unusual headache or dizziness should be discussed with a clinician.
Can NAD+ IV Make You Tired?
Yes, some people report temporary fatigue during or after NAD-related treatment. This can sound surprising because NAD+ is frequently marketed around “energy.”
But feeling tired afterward does not automatically mean:
- the infusion was effective;
- your body is “detoxing”;
- your mitochondria are “recharging”;
- or the treatment is producing a positive biological response.
Fatigue has many possible causes, including poor sleep, illness, dehydration, inadequate food intake, stress, medication effects, underlying medical conditions, or the treatment experience itself.
The FDA has also received adverse-event reports following compounded injectable NAD+ products that included fatigue, severe chills, shaking, and vomiting. In that context, FDA expressed concern that inappropriate food-grade ingredients used in sterile compounding may have introduced excessive endotoxins.

Chest Pressure, Throat Discomfort, and Increased Heart Rate
These symptoms deserve particular attention. The 2026 NAD+ IV pilot reported chest pressure, throat discomfort, increased heart rate, and congestion among participants during infusion.
Chest pressure or throat discomfort should never simply be normalized as something you need to “push through.” Tell the clinician immediately. Depending on the situation, treatment may need to be stopped, paused, slowed, or followed by additional medical evaluation.
Seek urgent medical care for severe chest pain, difficulty breathing, fainting, severe throat swelling, or persistent/rapidly worsening cardiovascular symptoms.
Are NAD+ IV Side Effects Caused by the NAD+ Itself?
Not necessarily. This is an important safety distinction.
A reaction following an IV infusion can potentially relate to several factors:
- the active ingredient;
- infusion rate;
- concentration;
- another ingredient in the preparation;
- contamination;
- endotoxins;
- IV placement;
- individual medical conditions;
- concurrent medications;
- or another unrelated issue.
FDA has specifically warned compounders about using food-grade NAD+ ingredients to manufacture sterile injectable products. The agency reported receiving adverse-event reports involving injectable NAD+ products, including severe chills, shaking, vomiting, and fatigue, with some patients needing medical treatment. FDA said the reactions were consistent with excessive endotoxin exposure.
In a separate 2026 warning letter, FDA described a compounded NAD+ lot associated with patients developing low blood pressure, uncontrollable shaking, chills, and body aches. Testing found very high bacterial endotoxin levels in an unopened vial from the same lot.
This is why product sourcing and sterile-compounding quality are part of the safety conversation, not just the dose or infusion speed.
What Does NAD+ IV Feel Like?
There is no universal NAD+ IV experience. Some clients may report little or no discomfort. Others may notice temporary sensations during the infusion.

A significant point for marketing NAD+ is that not everyone leaves feeling instantly energized. Immediate energy, focus, euphoria, or mental clarity should not be promised.
How Long Do NAD+ IV Side Effects Last?
There is not enough evidence to give one reliable duration for all NAD+ IV side effects.
In the small 2026 pilot study, infusion-related symptoms resolved when the infusion was completed. That does not mean every reaction will resolve immediately.
Duration may differ depending on the symptom, individual tolerance, underlying medical conditions, other medications, hydration, formulation, and the cause of the reaction. Persistent or worsening symptoms should be medically evaluated.
Do not assume that symptoms lasting hours or days are simply part of an expected “NAD detox.”
NAD+ IV vs. NAD+ Injections: Are the Side Effects the Same?
Not necessarily.
IV therapy delivers NAD+ directly into the bloodstream over a period of time. An injection uses a different administration route and exposure pattern. Therefore, you should not assume NAD+ injection side effects equal NAD+ IV side effects.
